Summer Camp is Postponed Until 2021!

Medical Questionnaire and Records

All medical records must be completed and be on file at Summer Camp by June 5. This includes an up-to-date record of a doctor’s physical examination and an up-to-date immunization record.

We recommend you START EARLY with this process. Specific directions are included in the medical questionnaire in the camp application form.

You must provide medical records as  doctors and other health professionals will  release medical records to a parent or guardian only.

Records may be faxed to Summer Camp at 845/452-8236 or emailed to  [email protected].

Medical records are required. Your child will not be admitted to camp on the first day unless we have a completed medical record  on file. 

What does a child need to bring to camp?

  • suitable play clothes
  • sturdy shoes for outdoor activities
  • bathing suit & towel twice a week.

Meals Provided

Note that breakfast, lunch and snack are provided by the Poughkeepsie City Schools Summer Feeding Program.
